29 CFR 1926.1203(d):  The employer decided that employees it directs will enter a permit space, and the employer did not ensure a written permit space program that complies with �1926.1204 implemented at the construction site. The written program must be made available prior to and during entry operations for inspection by employees and their authorized representatives.  Knauf Insulation Inc. work site area 621:  On or about February 8, 2017 and at times prior to, the employer did not ensure that a written permit confined space program was available and implemented on site while employees were conducting welding and grinding work tasks on stainless steel sheet metal in confined space areas.

29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:   a)Knauf Insulation, Inc. worksite:  On or about February 8, 2017 and prior to, employees were exposed to wood dust inhalation hazards from welding tasks.  The employer provides and allows voluntary use of NIOSH approved dust masks for welding work tasks and did not provide training appendix D training.
