29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts: On or about 02/01/17 an employee engaged in metal fabrication activities at the Accur-Shear Machine (#62506) was exposed to amputation hazards. The employee was exposed in that the guarding system was not adequately adjusted between the roller guards and table.
