The path to ground from circuits, equipment, and enclosures was not permanent and continuous: Thunder Valley Casino Room 930: There was no grounding prong on an orange flexible extension cord that powered a 110 VAC floor lamp which exposed employees to electrical shock hazards.

29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables were not protected from damage. Sharp corners and projections shall be avoided. Flexible cords and cables may pass through doorways or other pinch points, if protection is provided to avoid damage. Thunder Valley Casino Room 930: A twenty five (25) foot long flexible cord was not protected from damage where cord was being pinched between the door and the frame. Employees were exposed to electrical shock and burn hazards.
