29 CFR 1910.134(d)(1): The employer did not select and provide an appropriate respirator based on the respiratory hazard(s) to which the worker was exposed: On or about January 27, 2017, an employee was hospitalized as a result of an exposure to potassium cyanide. The employer did not ensure that employees wore the appropriate respirator for the task being completed. The employee was wearing a 3M 8511 NIOSH TC-84A-1299 N95 dust mask although the appropriate respirator to be worn would be a full face respirator with a P100 filter.

29 CFR 1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function: On or about January 27, 2017, the employer failed to assure that employees required to wear respirators while performing tests on incipient products using compendial methods; maintained a proper sealing surface of the face piece and face. Employees had facial hair that interfered with seal of the face piece and face causing an improper fit and as a result, an employee sustained health effects associated with exposure to potassium cyanide.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: On or about January 27, 2017, the employer failed to provide effective information and training to lab technicians who work with incipient products tested with compendial methods. An employee assigned to work with potassium cyanide was not trained on its hazard prior to his first exposure. The lack of training resulted in a the hospitalization of the employee, who sustained health effects as a result of exposure to potassium cyanide.
