29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems: (a) Flawless Steel Welding at 4824 Thompson Parkway Johnstown, CO: On or before 12/29/2016, the creating, exposing, and correcting employer did not ensure that employees decking the roof were protected from fall hazards by an adequate fall protection system. An employee who was installing the roof deck fell and sustained a fracture and a concussion. This condition exposed employees to fall hazards of approximately 19 feet.

29 CFR 1926.761(b): The employer did not train each employee exposed to a fall hazard in accordance with the requirements of 29 CFR 1926.761: (a) Flawless Steel Welding at 4824 Thompson Parkway Johnstown, CO: On or before 12/29/2016, the creating, exposing, and correcting employer did not ensure that employees decking the roof were trained regarding the fall hazards of the site. Employees were installing the leading edge of the decking, and were not trained on fall protection requirements. An employee fell and sustained a fracture and a concussion. This condition exposed employees to fall hazards of approximately 19 feet.
