29 CFR 1910.132(h)(1): The employer did not provide personnel protective equipment (PPE) at no cost to employees, except as provided by paragraphs(h)(2) through (h)(6) of this section: (a) Ultra Energy Solutions, LLC., at North Wall of Fox Creek, 34-1 Well Pad, Hereford, CO, 80735: The employer's hazard assessment identified and mandated the use of steel toed rubberized boots (Arctic Sport ST manufactured by Muck) when employees performed hydro-excavation work with a vacuum truck. These steel toed rubberized boots were not provided at no cost to employees.
