29 CFR 1910.184(c) (4): Slings were loaded in excess of their rated capacities: (a) BOS Solutions, @ Anadarko Rig X22, 2000 CR 23, Fort Lupton, CO 80621: On or before November 28, 2016, as part of injury/accident prevention responsibilities under its safety program, the exposing employer allowed the use of a short chain to be loaded in excess of its rated capacity. This chain was not designed for hoisting, and was used for lifting a shale metal bin onto a ground mat. The chain was rated in excess of its rated capacity, and snapped. This condition exposed employees to the hazard of being struck-by the chain and the falling load.

29 CFR 1910.184(c)(10): Hands or fingers were placed between the sling and its load while the sling was being tightened around the load: (a) BOS Solutions, @ Anadarko Rig X22, 2000 CR 23, Fort Lupton, CO 80621: On or before November 28, 2016, as part of injury/accident prevention responsibilities under its safety program, the exposing employer allowed an employee to place his hands and fingers between the sling and its load while the sling was being tightened around the load. The crew was moving a shale bin onto a ground mat, and an employee attempted to unhook the alloy steel chain sling from the forklift stinger. The employee placed his right hand between the chain and the shale bin, when the chain tightened around the load. This condition resulted in exposure to an amputation hazard and the amputation of a finger.

29 CFR 1910.184(e)(1): Alloy steel chain slings did not have permanently affixed durable identification stating size, grade, rated capacity, and reach: (a) BOS Solutions, @ Anadarko Rig X22, 2000 CR 23, Fort Lupton, CO 80621: On or before November 28, 2016, as part of injury/accident prevention responsibilities under its safety program, the exposing employer did not ensure that the steel chain sling used for lifting had permanently affixed durable identification stating size, grade, rated capacity, and reach. The short steel chain was used to lift a shale bin onto a ground mat, when the chain snapped. This condition exposed employees to the hazard of being struck-by the chain and the falling load.

29 CFR 1910.184(e)(3)(i): In addition to the inspection required by paragraph (d) of this section, a thorough periodic inspection of alloy steel chain slings in use was not made on a regular basis, to be determined on the basis of (A) frequency of sling use; (B) severity of service conditions; (C) nature of lifts being made; and (D) experience gained on the service life of slings used in similar circumstances. Such inspections shall in no event be at intervals greater than once every 12 months: (a) BOS Solutions, @ Anadarko Rig X22, 2000 CR 23, Fort Lupton, CO 80621: On or before November 28, 2016, as part of injury/accident prevention responsibilities under its safety program, the exposing employer did not ensure that thorough periodic inspections of alloy steel chain slings in use were made on a regular basis, to be determined on the basis of (A) frequency of sling use; (B) severity of service conditions; (C) nature of lifts being made; and (D) experience gained on the service life of slings used in similar circumstances. Employees were allowed to use alloy steel chains for lifting that had not been adequately inspected. This condition exposed employees to struck-by hazards. Note: Such inspections shall in no event be at intervals greater than once every 12 months.

Where such defects or deterioration are present, the sling were not immediately removed from service. A short chain was used to lift a shale metal bin onto a ground mat, and snapped.
