On 02/08/2017 an employee was exposed to a fall  distance of approximately 10 feet where the employee leaned out of a scissor lift (HY-BRID HB 1430) to push panels off a pile of panels onto a crane hook without using any fall protection device while he was not standing firmly on the platform of the scissor lift.
