On or about February 8, 2017, at the above addressed jobsite, an employee performing roofing operations from the roof was exposed to falls, of twenty (20) feet five (5) inches, created by the presence of unprotected sides.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a. On or about February 8, 2017, at the above addressed jobsite, portable ladders used to access the roof did not extend at least three (3) feet above the upper landing surface, thereby exposing employees to the hazards associated with falls.
