29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) Jobsite, 374 Theatre Drive, Johnstown, PA: On or about February 8, 2017, two employees were working along the edge of the building putting edge form on the exterior of the building wall and were not protected from falling. The fall hazard to the employees was 9 feet to the ground below.

29 CFR 1926.503(c)(3): Inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill: a) Jobsite, 374 Theatre Drive, Johnstown, PA: On or about February 8, 2017, three employees were working at the edge of the first floor of a commercial building and were exposed to a fall hazard of 9 feet to the ground below. The employer provided documentation that the employees had been previously trained in fall protection, but were not utilizing at the time of the inspection, indicating that they did not retain that knowledge.
