29 CFR 1910.178(l)(2)(ii): Training shall consist of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.  At the jobsite located at 12800 W 110th Terrace Overland Park KS:  Employees engaged in residential roofing activities using the Caterpillar TL10550 forklift were exposed to struck-by hazards in that forklift training had not been provided to include formal instruction and practical training.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At the jobsite located at 12800 W 110th Terrace Overland Park KS:  Employees engaged in residential roofing activities from a 4:12 pitch roof were exposed to fall hazards of approximately 10 feet from the eave of the structure to the ground below in that the ladder used to access the roof was not extended at least 3 feet above the eave and was not secured to the structure.
