29 CFR 1926.760(a)(1): Except as provided by paragraph (a)(3) of this section, each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level shall be protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems.  Harman Construction project site located at 6710 S. Business Dr. Sheboygan, WI: On February 9, 2017, employees were exposed to falls while installing metal roofing on an engineered steel building when fall protection systems or procedures were not used.

29 CFR 1926.761(b): Fall hazard training. The employer shall train each employee exposed to a fall hazard in accordance with the requirements of this section. The employer shall institute a training program and ensure employee participation in the program.  Harman Construction project site located at 6710 S. Business Dr. Sheboygan, WI: On and before February 9, 2017, employees performing steel erection activity had not been trained in the methods available for fall protection or the specific application of the equipment provided to them.

29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, as applicable:  29 CFR 1926.454(a)(2): The correct procedures for dealing with electrical hazards and for erecting, maintaining, and disassembling the fall protection systems and falling object protection systems being used;  29 CFR 1926.454(a)(4): The maximum intended load and the load-carrying capacities of the scaffolds used;   On February 9, 2017, employees were exposed to fall hazards while using aerial lifts to access the roof when they had not been training by a qualified person to properly use the lifts in that:  A: The Foreman operating the lift directed the three other employees to get onto the SkyJack SJ45T boom lift when the rated capacity was 2 workers or 500#;    B: The Foreman and Laborer using the SkyJack boom lift to egress the roof did not have lanyards to connect their harnesses to the lift;  C: Two employees climbed from the roof down onto the SkyJack scissor lift inside the building by using the guardrails as a ladder.
