29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  On or before February 9, 2017, employees were exposed to a fall of approximately 12 feet.  The employees were performing carpentry activities on a commercial building located in Cleveland, Ohio 44113.  The employees were working near a floor opening without proper fall protection.
