29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: On or about February 8, 2017, at the trenching site located at 3100 12th Ave. S.E. in Norman, OK, an employee was allowed to work in a trench more than 4 feet deep that had no ladder or other safe means access/egress. Employees were exposed to struck-by and crushed-by hazards.

On or about February 8, 2017, at the trenching site located at 3100 12th Ave. S.E. in Norman, OK, no protective devices were in place to protect an employee working in a trench that was more than 5 feet in depth, exposing the employee to struck-by and crushed-by hazards.
