Ground level of building being re-roofed: On or about February 09, 2017, an employee was exposed to a Struck-by hazard while operating a Yale Lift Truck, Model GLP080VXNGGE094, to move roofing material around the site. The forklift had been modified in that the original forks had been replaced with shorter forks and the capacity plate was not changed to indicate this modification.

The forklift operator had not been trained nor certified by his current employer. b) Ground level of building being re-roofed: On or about February 09, 2017, an employee was exposed to a Struck-by hazard while operating a JLG forklift, Model 10054, while elevating roofing material (sheetrock) to the roof of the building. The forklift operator had not been trained nor certified by his current employer.
