29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity: On 02/09/2017, an employee, working for Niehaus Building Services LLC at the Mercy Hospital addition, 1400 Hwy 61 South, Crystal City, MO, was required to work from a tubular welded frame scaffold with an inadequate inspection or no inspection by a competent scaffold person. The working level of this scaffold was only decked at approximately 67%, the decking was coated with a wet fireproofing spray material, and questionable safe access due to set-up arrangement and conditions.
