On February 1, 2017 located along the front area of 2028 Elderberry Dr. Tipp City OH residential 8 in 12 three story apartment / condo complex gable end roof deck, there was an employee performing roof sheeting whos retractable personal fall arrest system anchor point location and swing fall clear distance allowed the employee to be exposed to a fall in excess of 30 feet and contact the ground below, in that the employer did not install a fall protection system which complied with the requirements of Subpart M before employee(s) began the work that necessitated the fall protection.
