29 CFR 1910.303(g)(1)(i)(A): Working space for equipment likely to require examination, adjustment, servicing, or maintenance while energized shall comply with the following dimensions, except as required or permitted elsewhere in this subpart:  In the shop, the depth of the working space in the direction of access to the breaker box located to the rear of the air compressor was less than indicated in Table S-1 (three feet is minimum). Distances shall be measured from the live parts if they are exposed or from the enclosure front or opening if they are enclosed. Employees were exposed to a shock hazard and or the hazard of contacting the nip-points through the holes in the compressor guarding.

29 CFR 1910.178(l)(6): The employer did not certify that each operator had been trained and evaluated as required by paragraph (l) through a certification that included the name of the operator, the date of the training, that date of evaluation, and the identity of the person(s) performing the training or evaluation:  The employer did not ensure that the required evaluation/certification records/s were available for the employee/s who operated the Nissan Model 40 powered industrial truck.
