At the Sanctuary Subdivision jobsite, lot # 5, where sheathing was being installed on a 6/12 pitch residential roof, fall protection was not used exposing employees to a 9-ft. fall to the ground, on or about 2/09/2017.

At the Sanctuary Subdivision jobsite, lot # 5, where sheathing was being installed on a 6/12 pitch residential roof, fall protection was not used exposing employees to a 9-ft. fall to the ground.
