Lens Depot, Inc./East Brunswick, NJ- Employer did not ensure that aisles providing access to emergency exits were at least 28 inches wide at all points throughout the warehouse area:  a) Front warehouse area by the entrance to the office: aisle width varied between 27-25 inches.  b) Front warehouse area by the exterior wall: aisle width was limited to 19 inches as boxes were placed within the aisle.    Condition observed on or about February 13, 2017.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    a) Lens Depot, Inc./East Brunswick, NJ- Warehouse Area: An electrical Relocatable Power Tap (RPT), also known as an extension block,  power bar, plug board, and by many other variations, was used in an industrial setting and not as intended. Three Relocatable Power Taps were connected together in a sequence in order to provide power to an industrial fan and additional equipment.     Condition observed on or about February 13, 2017

29 CFR 1910.303(g)(1)(ii): Working space about electric equipment rated 600 volts, nominal, or less was used for storage:    a) Lens Depot, Inc./East Brunswick, NJ- Warehouse Area: The electrical panel boxes rated 600 volts or less were blocked by two pallets filled with items such as but not limited to a vacuum, parts for the vacuum and boxes with lenses. A minimum of 30 inches of space was not maintained in front of the electrical panel.    Condition observed on or about February 13, 2017.
