1926.760(a)(1): Except as provided by paragraph (a)(3) of this section, each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems. On 1/18/2017, employees from J Wilkinson, Inc. were installing decking on a roof of a commercial building located 2530 East pitman Street, O'Fallon, MO. The foremen and two employees were engaged in a steel erection activity on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level. Employees were not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems.

29 CFR 1926.152(a)(1): Only approved containers and portable tanks shall be used for storage and handling of flammable liquids. Approved safety cans or Department of Transportation approved containers shall be used for the handling and use of flammable liquids in quantities of 5 gallons or less, except that this shall not apply to those flammable liquid materials which are highly viscid, which may be used and handled in original shipping containers. For quantities of one gallon or less, the original container may be used, for storage, use and handling of flammable liquids. On 1/18/2017, Two plastic containers partially filled with gasoline were sitting alongside a wall next to the front loader which were used to fuel scissor lifts and other equipment. The two plastic containers were located inside of a commercial building located 2530 East Pitman Street, O'Fallon, MO. There were three employees exposed to this potential fire/explosion hazard.
