29 CFR 1910.212(a)(1): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Sizing Department- On or about February 5, 2017, the employer did not ensure that employees were adequately protected from rotating parts on equipment, such as the conveyor pulley on the Split Sizing Line when the conveyor belt had been removed during the sanitizing operation. On February 5, an employee suffered multiple fractures to the arm when it was entangled in the conveyor pulley.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) was not done in sequence to achieve energy control by performing the following steps as required by 29 CFR 1910.147(d)(1)-(6): 1. Turn off and shut down power to the machinery, 2. Physically locate and operate energy-isolating devices in such a manner as to isolate the machine or equipment from the energy source(s), 3. Affix lockout/tagout devices to each energy isolating device, 4. Verify isolation and de-energization of machinery have been accomplished prior to starting the maintenance and/or servicing work: a) Sizing Department- On or about February 5, 2017, the employer did not ensure that employees performing sanitation operations, including cleaning and removal of chicken trim caught in the rotating conveyor pulley on the Split Sizing conveyor utilized the established procedure to de-energize equipment. JCG Industries, Inc. dba Koch Foods Poultry Inc. was previously cited for a violation of this occupational safety and health standard 1910.147(d), which was contained in OSHA inspection number 1073993, citation number 1, item number 2, and was affirmed as a final order on January 7, 2016, with respect to a workplace located at 4404 W. Berteau Ave, Chicago IL 60641.
