29 CFR 1926.501(a)(2): Employees were allowed to work on surfaces which did not have the requisite strength and structural integrity: a. 1508 North Hancock Street, Philadelphia, PA 19122 - On or about Jan. 30, 2017, the employer did not ensure that temporary stairway could support employees safety. An employee was injured when a temporary stairs collapsed inside a home under construction. He fell approximately 20 feet and suffered serious injuries.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stairrail system along each unprotected side or edge: a. 1508 North Hancock Street, Philadelphia, PA 19122 - On or about Jan. 30, 2017, the employer did not install a stair rail system along the open sides of a temporary wooden stairway, exposing sub-contractor employees to a 1 to 20 foot fall hazards while using the open sided stairway to access to the fourth floor level.
