29 CFR 1910.252(b)(2)(iii): Workers and other persons adjacent to the welding area were not required to wear appropriate goggles to protect them from the rays when noncombustible or flameproof screens or shields were not used: The employer is failing to provide an adequate number of welding screens in the welding areas to protect employees from ultraviolet rays during welding operations. Nor are they requiring/providing appropriate personal protective equipment for employees adjacent to the welding operations in the facility. One instance, which occurred on or about February 14th, 2017 involved an employee was welding on an auger while two other employees were in the shop area without the protection of welding screens or personal protective equipment such as goggles in the facility located at 2610 Grand Avenue, Kearney, Nebraska.

29 CFR 1910.272(k): Receiving pit feed openings in the grain handling facility, such as truck or railcar receiving pits, were not covered by grates which had a maximum opening width of 2 1/2 inches: The employer is failing to protect employees from trip hazards in the facility. The grating covering the dump pit in the facility exceeds the requirements of this standard. Employees working in this area are exposed to injuries including but not limited to, sprains and strains to ankles and knees if they were to step into these gate openings. One instance, which was identified on February 14th, 2017 involved the grating on the dump pit in the main building located in the facility located at 2610 Grand Avenue, Kearney, Nebraska.
