29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed:(a) On or about February 7, 2017, at the Dapec breast processor, employees were required to lift the protective cover while performing cleaning operations (first rinse) while the machine was running without the chain being guarded, exposing employees to the hazard of being caught-in the chain and sprocket.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: On or about February 7, 2017, at the Dapec machine, employees performing cleaning operations were required to do the first rinse while the machine was running without locking it out, exposing employees to the hazard of being caught-in the breast processor installed on the machine.
