29 CFR 1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed: On or about February 13, 2017, and in times prior thereto, periodic inspections of lock out procedures for machines such as but not limited to , the APV Baker frozen dough machine were not preformed and documented. Workers in the cookie production area were exposed to the hazard of being caught in the rollers.

29 CFR 1910.263(f)(3): The back(s) of divider(s) were not completely covered or enclosed to guard against the hazard of moving parts: Rear cover(s) of the divider(s) were not provided with a limit switch to prevent machine operation while the cover(s) were open: Rear cover(s) of the divider(s) were not hinged to prevent complete removal: Rear cover(s) of the divider(s) were not provided with a limit switch to prevent machine operation while the cover(s) were open: Rear cover(s) of the divider(s) did not have a catch or brace so designed that it would not have release due to vibration or bumping and drop on an employee: On or about February 13, 2017, and in times prior thereto, the guard protecting the paper rollers of an APV Baker, frozen dough machine, had not been replaced. Workers in the cookie production area were exposed to the hazard of being caught in the rollers. Guards were not interlocked to prevent the machine from operating when they were not in place.
