29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:  a) 308 Poplar Street, Richland PA - On or about February 15, 2017, employees were not provided protective clothing such as gloves, apron, shields, or covers to protect them from heat and radiation hazards while using a propane torch to melt resin off an extruder.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment had been performed through a written certification that identified the workplace evaluated; the person certifying that the evaluation had been performed; and the date(s) of the hazard assessment:  a) 208 Poplar Street, Richland PA - On or about February 15, 2017, the employer did not verify through a written certification that the required workplace hazard assessment had been performed.  Employees operating equipment not limited to: plastic extruders; saws; and propane torches are exposed to chemical and radiation hazards and flying dust/debris.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
