29 CFR 1926.451(c)(3): Supported scaffold poles, legs, post, frames, and/or uprights were not plumb and braced to prevent swaying and displacement:    (a)  3rd FLOOR, WEST END - Employees were installing roofing metal from a custom built supported scaffold, and six (6) of the six (6) uprights supporting the scaffold ranged from approximately 2 to 5 degrees out of plumb, thereby exposing the employees to possible fall hazards of approximately 11 feet to the ground below, on or about February 13, 2017.    Abatement certification required within 10 days after abatement date. 29 CFR 1926.451(f)(12): Work on or from scaffolds was not prohibited during storms or high winds and a competent person did not determine that it was safe for employees to be on the scaffold:    (a)  3rd FLOOR, WEST END - Employees were installing 23 foot by 3 foot sections of roofing metal during periods of high winds, and the wind blew a piece of the metal roofing into an employee and knocked him over the railing of the scaffold he was working from, which caused the employee to fall approximately 11 feet to the ground and resulted in severe head and back injuries, on or about February 13, 2017.
