Sand Blasting Booth: On or about February 14, 2017, and at times prior, an employee working in the Sand Blasting Booth was exposed to continuous noise levels at 330% of the permissible daily exposure (8-hour time weighted average sound level of 90 dbA) during the 465 minute sampling period on February 15, 2017; exposure calculations included a zero increment for the 15 minutes not sampled.

29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:  a. Sand Blasting Booth: On or about February 14, 2017, and at times prior employees working in the Sand Blasting Booth were exposed to continuous noise levels at 330% of the action level for noise exposure (8-hour time weighted average sound level of 85 dbA) during the 465 minute sampling period on February 15, 2017 without a hearing conservation program in place. Exposure calculations included a zero increment for the 15 minutes not sampled.   b. Sand Blasting Booth: On or about February 14, 2017, and at times prior employees working in the Sand Blasting Booth were exposed to continuous noise levels at 330% of the action level for noise exposure (8-hour time weighted average sound level of 85 dbA) during the 465 minute sampling period on February 15, 2017 without an effective monitoring program in place. Exposure calculations included a zero increment for the 15 minutes not sampled.   c. Sand Blasting Booth: On or about February 14, 2017, and at times prior employees working in the Sand Blasting Booth were exposed to continuous noise levels at 330% of the action level for noise exposure (8-hour time weighted average sound level of 85 dbA) during the 465 minute sampling period on February 15, 2017 without an audiometric testing program in place. Exposure calculations included a zero increment for the 15 minutes not sampled.   d. Sand Blasting Booth: On or about February 14, 2017, and at times prior employees working in the Sand Blasting Booth were exposed to continuous noise levels at 330% of the action level for noise exposure (8-hour time weighted average sound level of 85 dbA) during the 465 minute sampling period on February 15, 2017 without a training program with regard to occupational noise exposure in place. Exposure calculations included a zero increment for the 15 minutes not sampled.

29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:    a. Clark Industrial Inc.: On or about February 14, 2017, and at times prior, employees were exposed to respiratory hazards while performing powder coating, sand blasting, and machining operations and wearing both tight fitting, negative pressure, elastomeric respirators as well as powered air purifying respirators (PAPR) without procedures for selecting the most effective respirator.     b. Clark Industrial Inc.: On or about February 14, 2017, and at times prior, employees were exposed to respiratory hazards while performing powder coating, sand blasting, and machining operations and wearing both tight fitting, negative pressure, elastomeric respirators as well as powered air purifying respirators (PAPR) without medical evaluations of employees required to use respirators.     c. Clark Industrial Inc.: On or about February 14, 2017, and at times prior, employees were exposed to respiratory hazards while performing powder coating, sand blasting, and machining operations and wearing both tight fitting, negative pressure, elastomeric respirators without fit testing procedures.    d. Clark Industrial Inc.: On or about February 14, 2017, and at times prior, employees were exposed to respiratory hazards while performing powder coating, sand blasting, and machining operations and wearing both tight fitting, negative pressure, elastomeric respirators as well as powered air purifying respirators (PAPR) without procedures regularly evaluating the effectiveness of the program.

Storage Rack: On or about February 14, 2017, and at times prior, employees were exposed to fire and explosion hazards from oxygen cylinders being stored in direct proximity to propane and acetylene cylinders.

Dipping Area: On or about February 14, 2017, and at times prior, employees were exposed to corrosive material including, but not limited to, D-Solve 1012, which has a pH of 12, while cleaning materials without proper training regarding the hazards associated with chemicals in the workplace.   b. Powder Coating: On or about February 14, 2017, and at times prior, employees were exposed to chemical hazards from products including, but not limited to, Tiger Drylac Powder Coatings 049/82830, while performing powder coating operations without proper training regarding the hazards associated with chemicals in the workplace.
