29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:      On or about March 23, 2017, at the mixing area of the above addressed jobsite, an energy control procedure used for preventing re-energization of a TEM VeMag/Cutter Divider was not audited to insure it was correct and being followed, exposing employees to a caught by hazard.    Bimbo Bakeries USA Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1012655, citation number 1, item number 2 and was affirmed as a final order on 4/13/2015, with respect to a workplace located at 3310 Panthersville Road, Decatur, GA.

29 CFR 1910.253(b)(4)(i): Oxygen cylinders were stored near highly combustible material, especially oil and grease; or near reserve stocks of carbide and acetylene or other fuel-gas cylinders, or near other substance likely to cause or accelerate fire.   On or about March 23, 2017 at the maintenance shop of the above addressed jobsite, employees were exposed to a fire hazard from an oxygen cylinder stored next to an acetylene cylinder while not in use.

29 CFR 1910.305(b)(2): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose:  On or about February 16, 2017, at the maintenance shop area of the above addressed jobsite, an electrical junction box lacked a cover plate, exposing employees to an electrical hazard.
