29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level.   a. Blue Dog Roofing jobsite located at 14089 Eckel Junction in Perrysburg, Ohio:  On or about February 17, 2017, the employer did not ensure employees working from the ladder jack scaffold were protected from fall hazards of 10 feet 7 inches to the ground below while using the scaffold to install ice guard around the south eave of the house.

Blue Dog Roofing jobsite located at 14089 Eckel Junction in Perrysburg, Ohio:  On or about February 17, 2017, the employer did not ensure the scaffold platform for the ladder jack scaffold, used by employees to install ice guard along the eave, were bridged one to another.

If the employer relies on training conducted by another employer or completed prior to the effective date of this section, the certification record shall indicate the date the employer determined the prior training was adequate rather than the date of actual training.  a.
