29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a) On and before February 17, 2017, at the Historic Darby Mansion located at 529 So Walnut St , Cameron MO., Workers engaged in re-roofing activities were using portable ladders which did not extend at least 3ft above the upper landing surfaces. As a result, the workers were exposed to fall hazards up to approx. 28 feet to the ground level.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems: (a) On and before February 17, 2017, at the Historic Darby Mansion located at 529 So Walnut St , Cameron MO., Workers engaged in re-roofing activities on a steep (12:12) pitched roof, were not protected from falling by the use of Personnel Fall Arrest Equipment / Systems. As result, workers were exposed fall hazards up to approx. 35 feet to the ground level. Midwest Lifetime Roof Systems, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 565318, Citation Number 1, Item Number 3 and was affirmed as a final order on 09-02-2012, with respect to a workplace located at 6902 NW 83rd St , Kansas City, MO 64152.
