29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a) At the jobsite, on or about February 13, 2017, employees performing masonry work while exposed to being struck on the head by materials and tools were not wearing head protection.

29 CFR 1926.451(e)(6)(ii): Integral prefabricated scaffold access frames did not have a rung length of at least 8 inches (20 cm): a) At the jobsite, on or about February 13, 2017, employees climbed the walk-through end frame, which had less than eight-inch long rungs, to access the tubular welded frame scaffolding platform that they were standing on while they were performing masonry work, resulting in a fall hazard.
