29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: A) Roof of building, on or about February 14, 2017, employees were removing shingles from a steep roof without the use of fall protection, exposing the employees to a fall hazard of 23 feet.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder: A) East side of building, on or about February 14, 2017, employees were using a metal multiple position ladder to access the roof of a two story building. The ladder was not long enough to extend three feet above the landing area and the ladder was not tied off at the top, exposing the employees to a 23 foot fall hazard.
