On or about February 16,2017 the employer allowed employees to operate a Dewalt DW716 (Serial #30208) Double Bevel Compound Miter Saw in the cut area to saw/ cut wood and allowed a build up of saw dust, receptacles were not covered or dust tight.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws. On or about February 16, 2017, the employer was allowing employees to use extension cords connected to electrical saws in the cut area that had outer sheathing and/or strained relief either missing or pulled away from the plug. a) An orange extension cord was connected to a Dewalt DWE 575 7 1/4" circular saw, with the outer sheathing missing from the plug end of the cord and was being used to cut wood planks. b.) An reddish- orange extension cord was connected to a Milwaukee (Sawzall) Reciprocating Saw, to cut nails and cut doors from moldings.
