29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge: a) Exit Door: On or about February 13, 2017 and at times prior thereto, the employer did not ensure an exit door was maintained in the operational status it was design for when a hasp and bolt assembly was installed to prevent the door from swinging open which exposed employees to fire and smoke related entrapment hazards.
