29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     (a) On or about February 2, 2017, and at times prior, Edgewood Construction, LLC., did not ensure that employees were protected from falls of up to 21 feet, while setting up trusses and installing "outlookers" on the fascia of a residential building. Employer did not ensure the use fall protection systems resulting in an employee falling 16 feet to the ground below. 29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    (a) On or about February 2, 2017, and at times prior, Edgewood Construction, LLC., did not ensure that a training program was provided to each employee to recognize the hazards of falling and the procedures to be followed to minimize these hazards. Employees required to work at heights of up to 21 feet on a residential building were not trained on fall hazards.    Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours:    (a) On or about February 2, 2017, Edgewood Construction, LLC. did not ensure that an in-patient hospitalization as a result of a work-related incident was reported to OSHA within 24 hours. An employee was hospitalized as a result of a high fall at a residential construction site. Employer notified OSHA 12 days after the incident.        Abatement Note: Abatement certification is not required for this item.
