29 CFR 1910.178(l)(3)(ii)(I): Other unique or potentially hazardous environmental conditions in the workplace that could affect safe operation. The employer had not trained sit-down forklift operators on loading dock safety procedures in that on February 10, 2017, in the West Loading Dock Area, a forklift operator was exposed to fall hazard when closing the overhead dock door by pulling a rope on the door and the forklift rear wheel located on the lip of the dock leveler came down when the driver pulled away from the dock causing the forklift to fall to the ground and his left hand being crushed by the frame of the rollover cage of the forklift.

29 CFR 1910.178(m)(4): The employer shall prohibit arms or legs from being placed between the uprights of the mast or outside the running lines of the truck. On February 10, 2017, In the West Loading Dock Area, a forklift operator was exposed to a caught in-between hazard when he pulled a rope on the overhead dock door to close it and by doing so reached outside the running lines of the truck.
