29 CFR 1926.850(a): Evidence in writing was not available that an engineering survey performed by a competent person had been performed prior to permitting employee(s) to start demolition operations:  a) Lanett Bleachery and Dye Works in Lanett, Alabama: On or about February 14, 2017 and at times prior thereto, the employer did not ensure an engineering survey was conducted and documented for any of the individual and adjacent structures to determine the condition of the framing, floors, and walls, and possibility of unplanned collapse of any portion of the structure which exposed employees to death and other injuries associated with falls or being struck-by and crushed-by collapsing and falling portions of the structure.

Each story of exterior wall and floor construction was not removed and dropped into the storage space before commencing the removal of exterior walls and floors in the story next below:  a) Five Story Building: On or about February 14, 2017 and at times prior thereto, the employer demolished portions of the steel framing, wooden floors, and brick walls on each level of the building from the bottom up which exposed employees to death and other injuries associated with falls or being struck-by and crushed-by collapsing and falling portions of the structure.

Employees were permitted to work where such hazards exist until they are corrected by shoring, bracing, or other effective means:  a) Five Story Building: On or about February 14, 2017 and at times prior thereto, the employer did not ensure bracing, shoring, or other effective means were used when a portion of the fourth level wooden floor was weakened by the removal of supporting structures which exposed employees to death and other injuries associated with falls or being struck-by and crushed-by collapsing and falling portions of the structure.
