29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition.  Employees engaged in machining parts were exposed to slip and fall hazards while working in the center of the shop in that puddles of coolant oil approximately three foot in diameter were on the floor around the lathes.

29 CFR 1910.37(a)(3): Exit routes must be free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level.  Employees engaged in machining parts were exposed to fire and smoke hazards in that the exit on the northwest wall was blocked by numerous items such as pallets, box fans and drums.

Maintenance check includes an internal examination, except extinguishers with stored pressure.  Employees engaged in machining parts and welding were exposed to fire and smoke hazards throughout the shop in that annual maintenance checks had not been conducted on portable fire extinguishers since 2007.

Employees engaged in using the bench grinder by the northwest section of the shop were exposed to struck by hazards in that the bench grinder was not anchored but instead was found to be setting on top of the Lincoln welder unsecured.
