29 CFR 1910.147(c)(4)(ii)(B): The energy control procedure did not clearly and specifically identify all specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: Bleach Plant - On or about February 16, 2017, the lockout procedure for the Bleach Plant Scrubber did not identify the scrubber bypass valve which needed to be partially opened before the manhole cover was removed. Failure to open the scrubber bypass valve resulted in the release of chlorine dioxide.

29 CFR 1910.119(f)(4): The employer did not develop and implement safe work practices to provide for the control of hazards during operations such as lockout/tagout; confined space entry; opening process equipment or piping; and control over entrance into a facility by maintenance, contractor, laboratory, or other support personnel: Bleach Plant - On or about February 16, 2017, the lockout procedure for the Bleach Plant Scrubber did not identify the scrubber bypass valve which needed to be partially opened before the manhole cover was removed.
