Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or were likely to cause death or serious physical harm to employees in that employees were exposed to being caught-between a sliding door and a wall.  On or about February 9, 2017, in suite number 148 of the Linac Room, the sensor of the overhead BEA Iris Pedestrian Presence Detector was partially blocked,  while simultaneously the touch sensors on the NB-4000 Linac Slider Door failed to signal to retract upon contact with an object, which allowed the NB-4000 Linac Slider Door, serial number i10-4-306-1m, to close without retracting, creating a caught-between hazard.    Among other methods, feasible and acceptable abatement methods to correct this hazard include the following:    1.  Ensure that the sensor of the overhead BEA Iris Presence Detector is not blocked.  2.  Ensure that the door is operating within the parameters specified in Chapter 10.1 of ANSI/BHMA A156.10-2011, Standard for Power Operated Pedestrian Doors, to protect users from Entrapment (caught-between) hazards in sliding doors.  3.  Implement procedures, as recommended by the manufacturer in the owner's manual, for monthly inspections of the safety features.  4.  Implement procedures, as recommended by the manufacturer in their training video, for daily inspections of the door and sensors.
