29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:     On or about February 17, 2017, during the installation of roofing material at the job site located at 7099 Emerald Bay Avenue NW, Massillon, Ohio, the employer did not ensure employees working on the roof of a new construction single family home were protected from possible danger of eye injury from impact, or from flying objects from the employees roofing material.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:     On or about February 17, 2017 at the job site located at 7099 Emerald Bay Ave NW, Massillon, Ohio 44646; during the installation of roofing material on a residential roof, the employer did not ensure portable ladders extend 3 feet above the upper landing surface.
