29 CFR 1926.403(i)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures, nor by any of the means listed in subparagraphs (A) through (D) of this paragraph: (a) At the jobsite, on or about 2/14/17: Workers were exposed to live electrical circuits when installing tile to a wall and when walking in a narrow passage between that wall and stored material with multiple live exposed circuits. On this wall was a 200 amp service panel cover and with open breaker spots, a second single switch open live breaker box, and an other open switchbox, also with current.

29 CFR 1926.403(e): Conductors were not spliced or joined with splicing devices designed for the use or by brazing, welding, or soldering with a fusible metal or alloy: a) At the jobsite, in the basement, on or about 2/14/17: Workers were exposed to live electrical circuits when working in the vicinity or when storing materials in close proximity to a ventilation fan that was spliced with a twisted connection and electrical tape.
