29 CFR 1926.102(a)(1): 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  At the residential construction site located at 1228 Buckingham Court in Raymore, Missouri:  Employees engaged in framing activities using a Skilsaw circular saw were exposed to struck-by hazards in that eye protection was not provided while cutting wood.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:  At the residential construction site located at 1228 Birmingham Court in Raymore, Missouri:  Employees engaged in residential framing activities were exposed to an electrical shock hazard.  The 110 VAC energized conductors were exposed on the female plug end of the extension cord providing power to the Skilsaw circular saw.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:  At the residential construction site located at 1228 Buckingham Court:   Employees engaged in framing activities using a Skilsaw circular saw were exposed to serious electrical injuries, in that the saw used to cut framing materials did not have a ground pin to ensure the path to ground was permanent, continuous and effective.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  At the residential construction site located at 1228 Buckingham Court in Raymore, Missouri:  Employees engaged in residential framing activities were exposed to an electrical shock hazard.   Strain relief was not provided on the cord providing 110 VAC power to the Skilsaw circular saw.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  At the residential construction jobsite located at or near 1228 Buckingham Court in Raymore, MO: Employees engaged in residential framing activities, walking the top plates, were exposed to fall hazards of approximately 17 feet from where the employees were working to the exterior ground below in that fall protection was not utilized.
