29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: (a) At the south side of the building where gutters where being installed with the use of a JLG aerial lift, model M600S, serial number 0300214872, no fall protection was provided exposing two employees to a 30 ft. fall hazard on or about 1/25/2017.
