29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6:  Employees were exposed to injury from a projectile hazard.  A compressed gas cylinder of Argon, that was not in use, was not secured to prevent damage in accordance as required by Compressed Gas Association Pamphlet P-1-1965 in the welding area.

29 CFR 1910.213(d)(1): Circular handfed crosscut table saw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut:  Employees were exposed to struck by hazards from an unguarded saw blade.  The employer did not provide a guard for the unmarked table saw used in the maintenance area.  The unguared saw blade extended 3 inches above the table.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:   Employees were exposed to a struck by hazard.  The employer did not properly adjust the work rest.  The improperly adjusted work rests were discovered in the following instances:  1) In the maintenance area, the Thor brand 6 inch bench grinder had its work rest adjusted to approximately 1/2 inch gap.  2) In the maintenance area, grinder #2, 6 inch pedestal grinder,  had its work rest adjusted to approximately 1 ½ inch gap.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  Employees were exposed to a struck by hazard.  The employer did not properly adjust the tongue guards.  The improperly adjusted tongue guards were discovered in the following instances:  a) In the maintenance area, the Thor brand 6 inch bench grinder was missing the tongue guard resulting in an approximate 1 inch gap between the abrasive wheel and the grinder housing, exposing employees to a struck by hazard.    b) In the maintenance area, grinder #2, 6 inch pedestal grinder, was missing the tongue guard resulting in an approximate 3 inch gap between the abrasive wheel and the grinder housing, exposing employees to a struck by hazard.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):  Employees were exposed to an electrical shock hazard from a damaged cord.  The employer did not inspect the cord to ensure it was free from damage.  The damaged cord was discovered on grinder #2 in the maintenance area.
