29 CFR 1926.20(b)(2):  The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of job-sites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has the authority to take prompt corrective measures to eliminate them):       (a)   Bristlecone Construction Corp. at 1300 N. Ogden St., Denver, CO 80218:  On and before February 17, 2017, the employer did not ensure each employee was protected from injury, in that carpenters were exposed to falls of approximately 10-12 feet without the use of an adequate fall protection system while constructing concrete forms and other work on the 2nd level of a residential condominium building under construction.

29 CFR 1926.501(b)(1):  Each employee on a walking/working surface having an unprotected side or edge which was six feet (1.8m) or more above a lower level was not protected from falling by the use of a guardrail system, a safety net system, or personal fall arrest system.      (a)   Bristlecone Construction Corp. at 1300 N. Ogden St., Denver, CO 80218:  On and before February 17, 2017, the employer did not ensure each employee on a walking/working surface having an unprotected side or edge which was six feet (1.8m) or more above a lower level was protected from falling by the use of a guardrail system, a safety net system, or personal fall arrest system in that  carpenters constructing concrete forms and other work on the 2nd level of a residential condominium building under construction were not utilizing a fall protection system. This condition exposed employees to a fall hazard of approximately 10-12 feet.
