29 CFR 1910.178(n)(4): Industrial truck driver(s) were not required to slow down and sound the horn at cross aisles  and other locations and wherever vision was obstructed:  (a) Shrink Wrap Department - On or about February 15, 2017, the employer did not ensure that forklift operators sounded the horn when moving double stacked pallets that obstructed their vision. An employee was pinned between two pallets when the pallets were pushed.   In accordance with the requirements of 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET)
