29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition: Workplace, 97 East 2nd Street, Unit C, Bayonne, NJ a) An aisle way leading from the warehouse area to a West emergency exit was created by stacking boxes of recycle materials in large boxes. These boxes had paper dust accumulations on them to depths of 1 inch. Analysis of the dust indicated that it was explosive. Violation occurred on or about 2-17-17

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition: Workplace, 97 East 2nd Street, Unit C, Bayonne, NJ Powered industrial trucks were operated with defects in the course of loading and unloading raw and processed recycling materials throughout the facility. The Caterpillar Model # EC 25 ST-12 operator right side signal light was damaged and not working, did not have a working horn, and was operated without a rear view mirror. In addition, forklift exhaust contained 506 parts per million (ppm) of carbon monoxide (CO). The Komatsu Model # EC 25ST-12 was operated with a horn that was not working. Violation occurred on or about 2-17-17
